Behavioral Health Consultant

The Center for Family Health is seeking a Behavioral Health Consultant to join our team. We are a Federally Qualified Health Center located in Jackson Michigan.  Our missions is: Opening the Doors to Healthcare for All.  We provide primary healthcare and dental services regardless of an individual's ability to pay.

Essential Duties and Responsibilities:

• Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
• Work with providers to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
• Use appropriate therapy to ensure patient's treatment goals are met with efficiency in the shortest possible timeframe.
• Provides information on public assistance, disability programs along with contact information on community resources, arrange transportation and other services related to medical care, assist patient with completing forms for various services.
• Assist in the detection of at-risk patients and development of plans to prevent further psychological or physical deterioration and assist in preventing relapse or mobidity in conditions that tend to recur over time.
• Teach patients, families and staff about care, prevention and treatment enhancement techniques.
• Conform with and abide by all regulations, policies, work practices and instructions.
• Lead patient education and support groups.

Experience/Education Required:

• Must have minimal Masters in Social Work or equivalent with limited or full licensure.
• Minimum of 2 years' experience in Social Work.
