Community Health Worker Level 1

The Center for Family Health is a Federally Qualified Health Center located in Jackson Michigan. Our mission is: Opening the Doors to Healthcare for All. We provide primary healthcare and dental services regardless of an individual's ability to pay.

The Center for Family Health currently has a position open for Community Health Worker Level 1. This is a full-time, non-exempt hourly position.


Position Summary:

The Community Health Worker will be responsible for working directly with patients and their families to help them navigate and access community services, other resources, and adopt healthy behaviors. This position will also be responsible for promoting and supporting patient and provider shared decision-making in pursuit of behavior and lifestyle modifications that improve health status and quality of life.


Essential Duties and Responsibilities:

  • Assists patients in identifying and securing health and human services.
  • Provides information and assistance with enrollment to public assistance, disability programs, and other various services.
  • Arrange transportation and other services related to health care.
  • Follows up with patients via phone, home visits and visits to other settings where patients can be found to assure comprehensive and coordinated care.
  • Provides crisis intervention support to patients by identifying issues related to patient health, to assist patients in securing and following through on mental and physical health care and assess crisis situations and intervene as appropriate to ensure patient safety.
  • Assist patients in navigating health care system.
  • Utilization management and patient care coordination in conjunction with hospital emergency department, health plans, etc.
  • Work cooperatively with other staff including providers in coordinating patient care and pursuing team-based approaches to service delivery.
  • Engage patients/families in the development/maintenance and activation of a comprehensive treatment and self-management plan.
  • Conform with and abide by all regulations, policies, work practices and instructions.
  • Assist with building relationships in the community with outreach and other similar services.

Education/Training/Experience:

  • Must have a high school diploma or equivalent.
  • Certification as a Community Health Worker or willingness to become certified.
  • Bilingual candidates strongly desired.

Job Knowledge, Skills & Abilities:

  • Adequate knowledge of the various resources available in the community for CFH clients.
  • Excellent communication skills both written and verbal.
  • Ability to interact respectfully with diverse cultural and socio-economic populations.
  • Ability to work effectively and independently with clinical and administrative staff as well as maintain external communications throughout the community.
  • Ability to represent the agency effectively in a variety of settings and with diverse communities. Demonstrates understanding and appreciation for diverse cultures.
  • Ability to work with MS Office Products software programs.
  • Routine testing/training on various job related skills and competencies.
  • Must have a valid driver's license, automobile insurance and a reliable vehicle.
