Full Time Registered Dietician

Responsible for providing nutrition and diet support and assistance to CFH patients.  

All Registered Dietitians are assigned to a Patient Care Team that is committed to "Comprehensive Team-Based Care" to meet the needs of our patients and our community. Each Patient Care Team includes providers, medical assistants, triage nurses, referral specialists, patient care coordinators, patient service reps, social workers and dieticians.

The team based approach is a whole person care approach that engages the patient in their health care and requires all team members to be effective and supportive communicators.  A high degree of integration and collaboration is required to meet the varying and changing needs of our patients.

Essential Duties and Responsibilities:

• Provides nutrition education, assessments and timely follow up to moderate to high risk patients.
• Provides nutrition/diet education through group or individual counseling and intervention to participants in conjunction with medical provider and patient treatment plan.
• Performs nutritional assessment and plan development. Develops diet program for clients.
• Develop, implement and evaluate interventions for pediatric, diabetic members, and other target populations of CFH along with interventions for other health-related areas.
• Assumes maximum application of Center services to clients by making referrals within the Center, working with providers to determine best course of treatment.
• Conform with and abide by all regulations, policies, work practices and instructions.
• Provides case management services to clients.

Non-Essential Functions:  Performs other duties as assigned.

Education/Training/Experience

• Must be a Registered Dietitian.
• Minimum of 2 years experience as a Registered Dietitian.

Job Knowledge, Skills & Abilities

• Adequate knowledge of the various resources available in the community for CFH clients.
• Excellent communication skills both written and verbal.
• Ability to work effectively and independently with clinical and administrative staff as well as maintain external communications throughout the community.
• Ability to represent the agency effectively in a variety of settings and with diverse communities. Demonstrates understanding and appreciation for diverse cultures.
• Ability to work with MS Office Products software programs.
• Routine testing/training on various job related skills and competencies.
