Quality Improvement Associate

The Center for Family Health is a Federally Qualified Health Center located in Jackson Michigan. Our mission is: Opening the Doors to Healthcare for All. We provide primary healthcare and dental services regardless of an individual's ability to pay.

We currently have an open position for a Quality Improvement Associate. This is a full-time, non-exempt hourly position.

Essential Duties and Responsibilities:

  • Identifies patients through health plans gaps in care reports, immunization database, the patient's medical record and the use of clinically integrated network registry.
  • Reviews care needs and validates these needs against the medical record.
  • Reaches out to patients with validated gaps in care.
  • Schedules appointments and makes transportation arrangements.
  • Reached out to patients who are newly assigned to health plans.
  • Supports quality outcomes and high reliability in PCMH population.

Education/Training/Experience:

  • Must minimum of High School Diploma.
  • Minimum of 1-year experience in an office setting.
  • Experience with all MS Office programs
  • Experience in healthcare Information Systems preferred.

Job Knowledge, Skills & Abilities:

  • Demonstrate initiative, ability to work with others, and good professional judgment.
  • Ability to work independently and organize time effectively.
  • Excellent written and verbal communication skills.
  • Strong organization skills and attention to detail.
  • Knowledge of and well-skilled in Microsoft Word, Excel, Outlook, and Power Point.
  • Coaching, mentoring, and teaching skills.
  • Ability to relate well to people from diverse ethnic and cultural backgrounds.
