Quality Improvement Specialist

The Center for Family Health currently has a position open for a Quality Improvement Specialist.  This is a full-time, salaried position with a full benefits package, paid time off, and paid holidays.

 

Position Summary:

Lead and assist work pertaining to planning, implementation and analysis of QI activities.

 

Essential Duties and Responsibilities:

  • Screens and assesses the quality of patient care and services documented in the medical record.                           
  • Makes recommendations to improve processes.
  • Participates and facilitates with committees on QI initiatives.
  • Improves processes of care and outcomes for defined populations.
  • Promotes evidenced based care delivery for disease specific populations
  • Supports department development and on-going quality improvement activities.
  • Perform performance improvement, process standardization, project management activities and facilitating.
  • Develops tools to support consistent evidence based practice.
  • Attains expert knowledge of the NCQA PCMH recognition standards, policies, processes and application requirements.
  • Develops and implements best practices to improve workflow related to the Medical Home standards.
  • Develops and implements a training program to align work flow and organizational "culture" with the NCQA standards.

 

Education/Training/Experience

  • RN Preferred
  • Experience in a healthcare of Information Systems
