School Health Center RN

The Center for Family Health is currently seeking an RN for our School Health Centers.  This is a Monday through Friday 7:30 a.m. to 4:00 p.m. position that includes a lunch break, and no weekends or holidays. 

Position Summary:

The Patient Care Coordinator is an active member of the patient care team working to improve individual patient and population health through care coordination and patient education/support activities.

Essential Duties and Responsibilities:

• Completes forms when requested by patients with pertinent medical information to be reviewed and signed by provider
• Maintains and coordinates the tracking of CFH health initiatives
• Provides patient education when requested and appropriate, individual and group
• Triage walk-in patients
• Referrals to other health care agencies.
• Participates in chronic care collaborative groups.
• Assist with durable medical equipment orders and certificates of medical necessity.
• Coordinate patient care with patient care team.
• Assist providers as directed with patient assessments, health maintenance, referrals and test ordering.
• Develops/compiles printed materials for achieving and maintaining overall health of patients, as well as those specific to heart disease, diabetes, hypertension, cancers, oral care, smoking cessation, fitness and nutrition
• Follow-up with patient to assure appointments are met and/or condition improves.
• Conform with and abide by all regulations, policies, work practices and instructions.
• Assist with patient care as needed
